Cancer Insurance

Benefits at a Glance

Employees of Cobb County can apply for a Cancer Care Plan with Aflac.  And with Aflac's cancer policy, employees can choose the type of coverage you want and pay for it through the convenience of payroll deduction. Because premiums are paid on a pre-tax basis, this saves even more tax dollars. Moreover, the policy pays benefits regardless of any other insurance you may have. Benefits are paid direct to employees, unless otherwise assigned.

Plan Details

Cancer Protection and Assurance Plans

Employees can choose between two levels of the Cancer Protection Assurance Plans. Each provides varying levels of benefits for the covered insureds. Both plans include a $500 Annual Building Benefit Rider and if covering children, include an additional $10,000/child for a first occurrence benefit Rider.

Accident Insurance

Benefits at a Glance

Benefits are paid direct to employees for on/off the job accidents that occur. Employees can choose to cover spouse/children as well.

Cobb County Government is rated an "A" class for this policy, meaning that accident rates are the lowest available for employees to obtain Accident Insurance Coverage.

Critical Care Protection - Specified Health Event

Benefits at a Glance

Aflac’s Critical Care Protection policy helps provide financial peace of mind if you experience a serious health event, such as a heart attack or stroke. You will receive a lump sum benefit upon diagnosis of a covered event with additional benefits to be paid for things such as a hospital confinement, ambulance, transportation, lodging, and therapy.

All benefits are paid directly to you, unless otherwise assigned, and can be used for any out-of-pocket expenses you have such as car payments, mortgage or rent payments, or utility bills. Aflac Critical Care Protection allows you to help protect the things you love the most from the things you expect the least.

Hospital Choice Plan

Benefits at a Glance

This plan provides added protection in the event of a hospital stay. Benefits are a dollar amount benefit based on the type of service. There are two plan levels from which to choose with rates being blended.  If employees have a prior Hospital Plan, Aflac encourages employees to review the benefit changes prior to conversion (which requires Underwriting even with the Option 1 + Extended Benefit Rider which would otherwise be Guarantee Issue for new policy holders).

Guarantee Issue:

Underwriting Required:

Option 1 includes the $1,000 Hospital Confinement Benefit, plus the Extended Benefit Rider and the Hospital Stay and Surgical Care Rider.

Short-term Disability

Benefits at a Glance

Cobb County Government offers employees Short-term Disability as a Supplemental, voluntary product through Aflac.

Guaranteed Issue

Active full-time employees can elect up to $5,000/ month, not to exceed 60% of earnings.

Simplified Issue

Monthly benefit amounts above the guaranteed issue amount. If you would like additional coverage beyond the guaranteed-issue amount, you may apply for additional coverage with simplified underwriting (subject to income requirements), not to exceed $6,000/month benefit.

Income protection

Provides income protection for Sickness or Accident when you can't perform the duties of your job.

  • Elect a benefit up to 60% of your pre-disability earnings.
  • Benefit period: Six (6) month benefit duration.
  • Maternity Benefits: included as an illness- 6 weeks or 8 weeks caesarian

Individual Whole Life

Outline of Coverage

Select coverage on yourself and/or your spouse
Employee Guarantee Issue $100,000 under age 50 and $50,000 51+
Spouse Conditional Issue is $25,000 under age 50 and the maximum issue is $50,000

Guarantee Issue and Policy Limits

Guarantee Issue and Policy Limits are below:
Employee Guarantee Issue is $30/week with maximum issue $40/week
Spouse Conditional Issue is $5/week with maximum issue $10/week
Children/Grandchildren is $3/week Guarantee Issue with maximum $5/week

Long Term Care Insurance Rider

Long Term Care Insurance is included as a rider on Each Whole Life policy for $10,000 or more.

Automatically included at initial offering to employees and spouses ages 18 - 70
For long term care facility, nursing home care or assisted living facility provides a maximum monthly benefit that is the lesser of 6% of the death benefit, less any policy debt at the end of the waiting period, or $3,000
For home health care or adult day care, provides a maximum monthly benefit that is the lessor of 4% of the death benefit, less any policy debt at the end of the waiting period; your actual monthly expenses; or $1,500

Benefits are payable once you have been receiving long term care for 90 days, subject to the conditions of the rider.
Premiums are waived while you are receiving benefits.
The benefit period maximum is 100% of the death benefit, less any policy debt at the end of the waiting period for each benefit period.
The rider is tax-qualified, which means that any benefits you receive will not be subject to income tax.

Pet Health Insurance discounts are available for Cobb County Employees. Simply call 855-270-7387 or go to www.metlife.com/getpetquote to get a quote.

Vision

United Healthcare Vision Coverage 

Cobb County Government offers employees vision insurance as a supplemental, voluntary product via United Healthcare and Spectera.  If you have questions about the vision benefit, contact Houze & Associates, Inc. at 800-523-7135 or enrollment@houze.org.

The plan allows for participants to receive service from both private practice providers as well as retail providers. Employees have the freedom to choose any provider. However, if a participant chooses a non-network provider they must submit a reimbursement request to the insurance company.

Vision Card/Providers

United Healthcare and Spectera do not send customized vision cards to participants but have a Vision Card that can be downloaded and printed for use.

To utilize the benefit:

  • Notify the provider that you are an employee of Cobb County,
  • Provide your Social Security Number or Employee ID,
  • Let your provider know you have United Healthcare and Spectera Vision Insurance.

Receiving your vision benefit is as easy as visiting your United Healthcare and Spectera Vision Providers. To locate providers, call 800-839-3242 or use the Vision Provider Locator.

Out-of-Network Services

Employees can receive services out of network but will be reimbursed based on plan limits. Co-pays do not apply. Kaiser Medical Members are not eligible to file out of network for the exam, due to the service being covered under the medical plan as a copay.

Laser Surgery

United Healthcare Vision has partnered with the Laser Vision Network of America (LVNA) to provide our members with access to discounted laser vision correction providers. Members receive 15% off usual and customary pricing, 5% off promotional pricing at over 500 network provider locations, and even greater discounts through set pricing at LasikPlus locations.

Genomic Life (Enrollment only available during Annual Enrollment)

Genomic Life is a unique program that empowers individuals to identify their genetic risk of hereditary cancers, cardiovascular and additional disorders in advance. The program helps address the challenges presented when dealing with cancer. Genomic Life solves the problem of awareness, access, and affordability to services that can help enhance prevention and survival related to a cancer diagnosis. The plan includes hereditary testing, cancer information line, 2nd opinion pathology review, nurse advocates, financial and clinical trial navigation, and ongoing support for ANY cancer.
